A medical colleague came and tried to catch the elevator before the doors closed, and got caught by the shoulders. The doors malfunction and the elevator raised with his shoulders trapped. It ended up decapitating him, just above the jaw, and the woman was stuck in the elevator with his severed head for X-number of hours, until they were able to get her out.

IRL I have to always be to the side of the elevator near the doors so I can get out quickly, I have always had this hangup that it would close on me or it would suddenly move and crush me.
NOW IT HAS ACTUALLY HAPPENED TO SOMEONE. I'LL NEVER BE ABLE TO REASON THAT IT CAN'T HAPPEN EVER AGAIN.

I also dislike elevators. I know that most of them have sensors so that kind of thing won't happen, but I've been afraid of bringing dogs on leashes. Imagine getting in with a dog, then closing the door but the dog runs out and its leash is stuck ... There'd be nothing you could really do, and the dog would end up squished or being hung, unless there was someone else outside who saw what was happening and took the leash off the dog. 
That's such a sad thought! Poor doggy.
